Final SAP Evaluation & Return-to-Duty Process – DOT Language:

This meeting will serve as your final SAP evaluation under the DOT return-to-duty process. During this session, we will review your completion of the education and/or treatment recommendations previously outlined in your initial assessment. This evaluation determines whether you have demonstrated compliance and are now eligible to proceed.

Upon confirmation of full compliance, I will issue a “Follow-Up Evaluation Report” verifying that you have successfully met the SAP recommendations. This will qualify you to move forward to Step 5: the Return-to-Duty (RTD) test, which must be conducted under direct observation by a DOT-compliant testing facility.

If you are not currently employed or lack an employer representative to initiate Step 5, I will refer you to a Consortium/Third Party Administrator (C/TPA) who can coordinate this process on your behalf.
